Chicago Family Case Management
Focuses on families from pregnancy through the first year of life. The goal of this program is to reduce low birth weight babies and infant mortality.
The Chicago Family Case Management program is provides outreach and coordination of medical health, oral health (dental) and social services for families with pregnant women or infants up to one year of age with medical, social, psychological, or environmental needs. The primary goals of the Family Case Management Program are to decrease infant mortality, decrease low birth rates, and assure access to medical and dental care to eligible families in the city of Chicago and south suburban areas.
Pregnant Women and Parenting Women with Infants/children up to 9 months may enter the program and remain in the program until the child is 1 year old.
The goal of the program is to ensure that clients have adequate medical care during and after their pregnancy and that their infants and children receive appropriate and timely immunizations and well child visits. They encourage families to have a medical home.
